Put on a face mask if you are venturing into a clinical setting with symptoms.

          Let’s talk about microneedling. We have been performing microneedling procedures for quite some time in our clinic , and this technique has been around even longer since the 90’s. We have noticed it continues to be a very popular requested service by our patients.
         &#xD;

          For those of us unfamiliar with the technique of microneedling and its benefits,… you are missing out! So let’s break down what microneedling is all about.
         &#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;b&gt;&#xD;
      
           What is microneedling?
          &#xD;
    &lt;/b&gt;&#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    
          Microneedling is the practice of inserting tiny needles into the skin superficially to create small holes in the skin in a controlled fashion. It is also known as a Collagen Induction Technique, a non-invasive skin rejuvenation technique with little down time, and excellent results that boosts the deposition of collagen in the skin. Microneedling can be done at home with various at home devices (eg. Dermaroller) available over the counter, but when done in the clinic, can often be done in a more controlled, consistent, safer, cleaner way, and create better results with less potentially unwanted side effects.
         &#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;b&gt;&#xD;
      
           What are the benefits of microneedling?
          &#xD;
    &lt;/b&gt;&#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    
          Microneedling can be used to freshen up skin that has laxity, fine lines, fine wrinkles, acne scars, facial scars, and signs of aging skin. Clinic appointments for microneedling sessions can range from $200-$700 depending on the equipment, operator, and training of the operator. You can also combine microneedling treatments with potent anti-aging product application post procedure, or combined with PRP (platelet rich plasma) for a more bloody but more amazing end result! Microneedling has also been touted as a booster for its effects on hair growth, especially when combined with PRP treatments for hairloss or alopecia. This technique is also suitable for any skin types, including darker skin patients who are typically not good candidates for laser resurfacing treatments.
         &#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;b&gt;&#xD;
      
           How is it done and what is the recovery like?
          &#xD;
    &lt;/b&gt;&#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    
          The procedure is done in the clinic, with the use of a topical anesthetic agent, or a numbing gel, to help our patients stay comfortable during the procedure. The procedure can be performed on the face, neck, scalp, chest, hands, wherever rejuvenation is needed. The procedure lasts approximately 20 minutes, and the sensation is less like needles, but more prickly of a sensation. Needles used in this procedure range 30-32 gauge in size, and can penetrate the skin ranging from 0.2 to 4 mm depth depending on the device and skin need. The procedure can be repeated every 4-6 weeks, with most optimal improvement being noticeable after 3-4 sessions. Even after one session, you will notice that the skin is overall more glowing, smooth, bright with a more improved texture.
         &#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    
          To allow your skin to hold on to the replenished collagen from a microneedling session, it is best to periodically get microneedling sessions done throughout the year.

&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;b&gt;&#xD;
      
           Where should I go to get it done ?
          &#xD;

  &lt;p&gt;&#xD;
    
          I recommend that you get this procedure done at a medical facility trained to do this procedure safely for your skin. Supervision with a dermatologist or physician on site is preferred in case adverse reactions happen. Following treatment, you should be counseled on how to keep the areas clean and protected from too much sun exposure. There will be open channels on your skin that remain open after the procedure for a short period of time. I always advise my patients to avoid the gym or other questionable situations after the treatment to avoid complications and unwanted side effects. It is common to take advantage of the open microneedling channels and apply post procedure topicals and healing masks after a microneedling session so your skin can soak up all the antioxidnats, nutrients, and hydration that is beneficial for your skin. Redness and puffiness will resolve in the next 2-7 days.

          Few things are as distressing as a telephone call from your dermatologist saying that you have been diagnosed with a melanoma, and that you need to come in for further discussion. I hope this blog will be able to provide helpful explanations and guidance for patients who are in this situation. While melanoma is not the most common skin cancer that is found in the US population, it is one of the most dangerous skin cancers. Most melanomas are detected early and can be treated. However, fatalities from melanoma can happen when detected late. It is estimated that we will lose 6,850 people from melanoma this year in the US.
         &#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    
          What should you expect at the initial doctor visit after your skin biopsy to discuss a melanoma finding?
         &#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    
          Although all this info may sound scary, the statistics has shown that the odds are with you. 83% of all melanomas are diagnosed at Stage ZERO or 1. The relative survival rate of localized melanoma of the skin is
          &#xD;
    &lt;b&gt;&#xD;
      
           99%.
          &#xD;
    &lt;/b&gt;&#xD;
    
            Meaning if your melanoma is detected in the skin and has not spread, you have a 99% chance of living 5 years or longer after diagnosis (after 5 years, it is considered a CURE in the cancer world and low risk of this cancer causing harm).
         &#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    
          If you have a Stage ZERO or 1 melanoma, the standard treatment involves removing more skin around the biopsy site, with a surgical procedure that can be done on an outpatient basis, most of the time this can be done with patient being awake.  Some complicated locations, especially on the face, may require Mohs surgery (a special tissue sparing technique), or plastic surgery involvement.
         &#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    
          For those with early stage melanomas, this surgery (referred to as a “wide local excision”) may be the only treatment needed for the melanoma.
         &#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    
          For those who have advanced melanoma, lymph node testing, imaging scans, and more complex treatment regimen will be discussed. Every treatment plan will be different depending on the patient’s age, health history, and personal preferences. A referral to a university hospital may be needed for more complex treatment of advanced melanomas.
         &#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;

                    Acne is a multifactorial disease that can vary from mild breakouts to severe, scarring lesions. This is one of our most commonly seen problems in dermatology. It affects all ages, genders, and races. Most of the population considers it a “teenage problem” but it can occur at any point in one’s life, and it is not the result of poor hygiene.
                  &#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    
                    So, what causes acne?
                  &#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    
                    It is an inflammatory response within the pilosebaceous unit that is made up of a hair follicle and oil gland. Within this unit, there is a build up dead skin cells, increased oil production, and overgrowth of bacteria which all leads to inflammation. This process causes a variety of skin manifestations, such as whiteheads, pustules, papules, and cysts, depending on the severity. Acne can be worsened by hormones, trauma (a.k.a. squeezing and picking at pimples), environment (things such as makeup, irritating facial products or lotions, wearing masks and helmets), and diet. Diet has always been an area of interest and there is a known link with low-fat dairy products. A recent publication in JAMA Dermatology discussed an association (NOT causation) with adult acne from milk, along with sugary beverages and fatty foods.
                  &#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    
                    There are different types of acne:
                  &#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    
                    Treatments include a mixture of over the counter products and prescription medications. All treatment starts with a consistent routine of washing your face morning and night. After that, it depends on the type of acne you have and your skin type (e.g. sensitive, oily, dry), as this will determine what type of products you will be able to tolerate. Listed below are typical (but not complete) items dermatologists use to treat acne.
                  &#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    
                    Mild acne can usually be treated with over the counter products. Some of these include face washes that have active ingredients like salicylic acid, benzoyl peroxide, or sulfur. There is one prescription-strength retinoid that is now found in drug stores, Differin (adapalene 0.1%) gel. Other drug store brands carry retinol products (will need another blog post to discuss those differences!) and are a good start if you are new to retinols/retinoids. Starting a new skincare regimen can be irritating, cause dryness, so definitely do not skip moisturizing after your AM and PM routine (preferably a non-comedogenic one). Topical treatments take 8 weeks of consistent use before seeing significant improvement. However, most inflammatory acne, and certainly nodulocystic and hormonal acne, will require a prescription at some point. Do not delay in seeking professional medical help as acne scarring can be difficult to treat.
                  &#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    
                    A question that frequently comes up: how do I get rid of the dark spots?! This is called post inflammatory hyperpigmentation (PIH), and is not permanent scarring. It is the result of inflammatory acne as it causes excess production of melanin. This is commonly seen in skin of color due to the increase of melanin and sensitivity of melanocytes. This can be disheartening as it may takes weeks to months for the pigmentation to fade on its own. Most patients seek assistance with speeding up this process. General guidelines include upkeep of skincare routine to prevent further acne and daily sunscreen (physical blockers, zinc or titanium oxide, provider better protection) to prevent darkening from UV rays. From there, we can add a variety of topicals to help aid in the lightening of dark spots, along with chemical peels for more aggressive treatment. Ingredients we often use for PIH are salicylic acid, glycolic acid, azelaic acid, retinoids, and hydroquinone. Other topicals includes vitamin C, kojic acid, arbutin, and resorcinol. Working with your dermatologist, you can find a routine that fits your skin type and budget.

                    You may have heard that we are opening our clinic doors again to see our patients this week. While we focus on taking care of your skin concerns in clinic, we also will be implementing new protocols to keep the clinic environment as safe as possible. Part of that is done by educating ourselves with the right knowledge about this infectious pandemic.

                    So here are a few myths and facts that can help us understand what is true and what is not about COVID-19:

  &lt;p&gt;&#xD;
    
                    1. Myth: You can protect yourself from COVID-19 by injecting, swallowing, bathing in or rubbing onto your body bleach, disinfectants or rubbing alcohols.
                  &#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    
                    These products are highly toxic and should never be swallowed or injected into the body. Call 911 if this occurs and correct others that believe this absurd suggestion.
                  &#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    
                    2. Myth: A vaccine to cure COVID-19 is available.
                  &#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    
                    There is no vaccine for the new coronavirus right now. Scientists have already begun working on one, but developing a vaccine that is safe and effective in human beings will take many months.
                  &#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    
                    3. Myth: The new coronavirus was deliberately created or released by people.
                  &#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    
                    Viruses can change over time. Occasionally, a disease outbreak happens when a virus that is common in an animal such as a pig, bat or bird undergoes changes and passes to humans. This is likely how the new coronavirus came to be.
                  &#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    
                    4. Myth: Eating garlic can help prevent infection with COVID-19
                  &#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    
                    Garlic will help only because it makes social distancing certainly easier! There is no evidence that shows garlic protects people from the virus.
                  &#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    
                    5. Myth: Antibiotics can kill the virus that causes COVID-19. No antibiotic can kill a virus. Antibiotics are only to be used when there is known, documented co-infection with bacteria.
                  &#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    
                    6. Fact: COVID-19 is

                    &#xD;
    &lt;/b&gt;&#xD;
    
                    
  
  
     the same as the flu.
                  &#xD;
  &lt;/p&gt;&#xD;

                    Although both COVID-19 and seasonal influenza are both respiratory illnesses, they have many differences. They share some of the same symptoms, but symptoms usually appear more quickly with the flu than with COVID-19. The fatality rate from COVID-19 is much higher.
                  &#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    
                    One other key difference: There is a vaccine and specific medicines to treat the flu. There is currently no vaccine or medication for COVID-19.

  &lt;p&gt;&#xD;
    
                    7. Fact: People of any age should worry about COVID-19, not just the older population.
                  &#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    
                    COVID-19 can affect people of any age. It is true that older adults have a higher risk of getting COVID-19 and of having more serious complications from it. However, people of all ages can get the disease. It’s important for everyone to use prevention tactics such as handwashing and masks. Other at-risk groups include people with existing medical conditions like chronic lung disease, asthma, heart disease, liver disease, or renal failure, people who are immunocompromised due to factors like cancer treatment, HIV, transplants, and other causes
                  &#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    
                    All those factors can put people of any age at risk.
                  &#xD;

                    8. Myth: Hand dryers can kill COVID-19 No, hand dryers are not a treatment to kill the virus that causes COVID-19. The reason for that is that they just increase the air circulation of the viruses, as that air ― either warm or not warm ― is blowing against the skin of your hands.

  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    
                    9. Myth : Hot, humid climates protective you from COVID-19
                  &#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    
                    Evidence to date actually shows that COVID-19 can be transmitted in all areas, and climate has not been shown to be a factor.
                  &#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    
                    10. Myth: Mosquitoes can spread coronavirus
                  &#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    
                    Some people believe mosquitoes can spread the virus, but that’s not the case. Humans are spreading the coronavirus to each other via airborne droplets, and no mosquito is needed as a vector.

                    June 1-7, 2020 was Hidradenitis Suppurativa awareness week and I would like to provide some useful self-care tips for those who are living with or caring for someone with Hidradenitis Suppurativa (HS). The last few months with this COVID-19 pandemic have been anything but routine and normal for all of us, and this undoubtedly pose a bigger risk of stress and flare ups for those of us with chronic skin diseases. Our structured days become tremendously different in lockdown, and with these changes comes a level of anxiousness and stress that can lead to worsening of our skin’s predispositions. I hope with these tips and tricks we can help alleviate some of these stressors in order to keep your skin wellness a priority.

        What is Hidradenitis Suppurativa?

                    HS is a debilitating, chronic, complex systemic inflammatory skin disease. It can have a huge impact on one’s quality of life physically, mentally, and emotionally. HS consists of extremely painful reccuring bumps or abscesses that can develop anywhere hair follicles are found. Hair follicles are present throughout the body with the exception of palms of our hands, soles of feet, and lips. Therefore, it is possible to get hidradenitis lesions anywhere on the body.

  &lt;p&gt;&#xD;
    &lt;b&gt;&#xD;

        Who gets HS?

  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;

  &lt;p&gt;&#xD;
    
                    Majority of patients getting HS are women ( 3:1 ratio in women :men) , although men can also get HS. Family members with HS seems to be found in 10% of HS patients. Smokers, unhealthy lifestyles and being obese or overweight are often factors found in patients with HS. There is also a predisposition for African American patients diagnosed with HS over other ethnic backgrounds.

&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;b&gt;&#xD;

        What does HS look like?

  &lt;/p&gt;&#xD;

                    HS can look like acne, boils, tunnels in the skin, or painful lumps on the skin. Many patients are diagnosed initially as acne or infections. It takes an average of 7 years for an HS patient to get the correct diagnoses, thus consulting a dermatologist is key in this regard.
                  &#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;b&gt;&#xD;

        What can you do to prevent HS from becoming severe?

                    Early diagnosis and correct management is key. Some lifestyle changes can be adopted early and may prevent worsening course of HS. Some key lifestyle changes that will improve HS :

  &lt;p&gt;&#xD;
    
                    Your doctor can help assess your HS, correctly diagnose you if it is or is not HS, and advise on interventions that can be done to help with symptoms. Medical interventions may include prescription washes, ointments, and oral pills or injections. Surgical interventions can also be done to remove areas of HS.

Be it the lack of sleep, the stress of this pandemic, or way too much screen time and webinar attendance, they all can contribute to eye puffiness and make the lines and discoloration around the eye more visible. Although there are many eye rejuvenating procedures a board-certified dermatologist can offer at the office, there are also things you can try at home to soothe the eyes. This way, by the time you come in to our office for procedures, you will already be on a maintenance routine that can provide the most optimal outcome!

                    What causes puffy eyes and dark circles anyways? The answer is not so simple. Many factors can contribute to tired looking eyes. It is important to recognize what the dominating reasons are in each person. Everyone is unique and so are your eyes. Treatment recommendations are tied to the reason behind the appearance of the eyes. Each person will have a different combination of reasons for their “tired eyes” and thus the treatment recommendations for each person will be a bit different.
                  &#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    
                    When assessing eyes in the mirror for puffiness, try to do it first thing in the morning. Eye puffiness will often be at its worst in the morning and gradually improve during the day. Several factors can play a role. Those who suffer from airborne allergies, have a lot of congestion in the eye areas that lead to puffiness. This puffy look can cause a bluish look of the veins that become more visible. Lack of sleep can also be blamed for this. Treatments aimed at reducing these issues include using cooling agents such as cold rollers, metal-tip applicators for eye creams, soothing gel patches for the eyes, and a daily antihistamine such as Zyrtec or Claritin for those who suffer from allergies. Those who have deeply pigmented skin, may notice more pigment or darker color around the eye that is physiologic, or completely normal given their heritage.  Then for those with airborne allergies, the frequent eye-rubbing leads to thicker skin around the eyes. This too can lead to a darker color appearance of eyelid skin. Age can also lead to increased breaking down of collagen, and when this occurs around the eye, it leads to thinner skin in this area giving it a more “crepey skin” appearance.                     First, what is PPE?

                    This stands for personal protective equipment. Currently, we think of PPE as the supplies worn in hospitals by healthcare workers when caring for patients with COVID-19. This includes masks (N95 and surgical), goggles/glasses, face shields, hair coverings/caps, gowns, and gloves. However, PPE is worn daily in various occupations. It is: gloves worn by our sanitation workers; face shields worn by welders in car assembly plants; goggles worn in your chemistry glass; masks worn by painters; ear plugs and hard hats worn by construction workers. All of these different articles are considered PPE.

  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    
                    With COVID-19, the issue of PPE has come into the spotlight as hospitals continue searching for enough supplies and most governors are urging individuals to wear some when they are in enclosed public spaces. The continued use of PPE has already been wreaking havoc on the skin of doctors, nurses, and hospital staff. I’m sure the general population will be next. The most commonly seen skin reactions are marks and indentations from wearing a mask for long hours, dry and irritated skin, and acne flares.
                  &#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    
                    Here are some simple tips for taking care of your skin while using PPE for both healthcare workers and quarantiners alike!
                  &#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;b&gt;&#xD;
      
                      
    
    
      Keep up your routine!

     Maintaining your skincare regimen will help combat the negative effects of wearing a mask and prevent acne from flaring. You may just need to alter your normal routine a bit – keep reading!
                  &#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;b&gt;&#xD;
      
                      
    
    
      Use a gentle face wash.
    
  
  
                    &#xD;
    &lt;/b&gt;&#xD;
    
                    
  
  
     Wearing a mask can cause increased dryness of your facial skin and irritation, even microtears, from rubbing the same area over time. A gentle face wash, such as Cetaphil or CeraVe Hydrating Cleanser, will cleanse your skin without drying it out even more. Avoid harsh exfoliating washes, especially those with the abrasive scrubbing particles in them (I see you St. Ives Apricot Scrub users!), as these can worsen an already irritated face. Now may also be the time you hold off on or reduce the frequency of using your retinol/retinoid because they are a chemical exfoliator.
                  &#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;b&gt;&#xD;
      
                      
    
    
      Lather on the moisturizer.
    
  
  
                    &#xD;
    &lt;/b&gt;&#xD;
    
                    
  
  
     Some people skip this step in the morning because they feel they get oily throughout the day. However, lotion not only moisturizes your skin but gives you a layer of protection from the environment. This will help protect your face from the dryness and irritation caused by a mask.
                  &#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;b&gt;&#xD;
      
                      
    
    
      Lighten up on the makeup!
    
  
  
                    &#xD;
    &lt;/b&gt;&#xD;
    
                    
  
  
     Heavy foundation already clogs the pores but putting a tight fitting mask on top – hello breakouts! You can continue wearing eye makeup but maybe skip the foundation, concealer, bronzer, blush, highlight – no one will be seeing that underneath a mask anyway. Perhaps opt for a tinted moisturizer, or even better, a tinted sunscreen!
                  &#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;b&gt;&#xD;
      
                      
    
    
      You may need a barrier.
    
  
  
                    &#xD;
    &lt;/b&gt;&#xD;
    
                    
  
  
     For those having to wear a mask, especially the N95, for prolonged hours at work this tip is for you. Applying a barrier on high contact areas of your skin (bridge of nose, cheeks, chin, forehead, behind the ears) before putting on your mask and goggles can help with the discomfort. This can be as simple as a layer of Vaseline or Aquaphor ointment. For those wearing this PPE daily, you may need a stronger barrier, such as Triple Paste cream, applying paper tape, or a gel bandage like DuoDERM. Be advised that using the thicker barriers may affect the seal of your N95 and would recommend retesting the fit.

&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;

                    May is Skin Cancer Awareness month! In the spirit of this national awareness month, we at Dermatology Specialists of Brighton would like to educate our patients and community on how to look for and prevent dangerous skin spots while we shelter in place. Our practice offers virtual appointments for focused skin concerns, but we realize that a total body skin exam cannot be practically done in a virtual visit setting. So until we can see you safely in our offices again, and restrictions of shelter in place has been lifted or relaxed significantly, here are 5 steps you can do to help prevent and detect spots suspicious for skin cancer:
                  &#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    
                    Did you know that women who tan indoors before the age of 30, are 6 times more likely to develop melanoma- the deadliest form of skin cancer? Just one indoor tanning session can increase the risk of developing melanoma by 20%, squamous cell carcinoma by 67%, and basal cell carcinoma by 29%. For a glowing tan color in your skin, opt for the self-tanner that comes as wipes, creams, or lotions, and take the smarter option this way instead of taking the risk of skin cancer.
                  &#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    
                    The sun’s rays are strongest during these times of the day between 10 am and 4 pm. Even on cloudy days, up to 80% of sun rays can still reach your skin after penetrating cloud coverage. If you are around snow surfaces, an additional 80% of rays can be reflected off the shiny surface. Sand also can reflect 20% of sun rays, so be careful to reapply sunscreen even under the shade.
                  &#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    
                    Even one blistering sunburn in one’s past can double their risk to develop melanoma later in life. Be careful to pick the correct sunscreen for your outdoor activities. Wear a broad spectrum sunscreen that blocks UVA and UVB, with an SPF of 30 or greater, with a physical blocker such as zinc or titanium dioxide. Reapplication is important since sweat and water activities can decrease the amount of sunscreen that remains on our skin. Apply sunscreen to all exposed areas of your skin including the ears, your lips (there are lip balms, lip glosses with SPF 30 sold in drugstores and in our office!), back of the neck, and tops of your feet.

  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;

                    Looking for ABCDEs means looking for asymmetry, irregular borders, multiple colors, diameter bigger than the tip of a pencil eraser, and evolution in any of your moles. Those are the characteristics of a mole that may be a skin cancer spot. Look for those signs when you do the following

     will be a good source when looking for the above ingredients. Make a habit of reading a product’s ingredient list on the back of the bottle to find out about its main active ingredient. Don’t just buy a product because of their advertising statements.   4. Modify your diet to avoid too much dairy, sugar, and processed food items. Limit protein powder intake, or eliminate completely if possible.
                  &#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    
                    5. Do not pick your pimples, do not scrub hard when washing the face, and let things heal naturally. Resist the temptation to “pimplepop”.   6. Do not use tanning beds to clear your acne.
                  &#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    
                    7. Don’t torture your skin with toners, astringents, harsh detergents, or alcohol solutions.   8. Be patient, and give your skin about 6-8 weeks at a chance for improvement when starting over the counter options.
                  &#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;

                    Uneven complexion and blemishes on the face are very common skin concerns that patients bring up during cosmetic consultations with me. As a board-certified dermatologist in Brighton, Michigan, I have multiple clinic lasers and procedures that I can use to help brighten one’s complexion. However, I always remind patients that there are many good over the counter products as well that can be tried before committing to procedures and prescription products. Here are a few items to look for if you want to even up a blotchy complexion with over the counter products:
                  &#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    
                    You should always understand the science behind why blemishes happen on the face.  Dark uneven complexion on the skin happens from the presence of too much melanin and inflammation in the skin. Melanin is produced by pigment producing cells called melanocytes in the skin. Melanocytes are activated by ultraviolet light (UV, found in sunlight), and makes more color in response to it. To a lesser extent, visible light and infrared light too can activate melanocytes.
                  &#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    
                    To prevent this activation of melanocytes, reducing or avoidance of UV light, infrared, and visible light can help in reducing eventual pigmentation, and to maintain improvement from a successful regimen. The use of facial sunscreen for this purpose should be daily, regardless of season, enough of the product should be used, and reapplication during the day should happen if possible.
                  &#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    
                    The best daily sunscreens on the market that would not irritate even those with sensitive skin types, are light weight mineral based or physical sunscreens with a tint. Sunscreen brands with a good reputation include : Avene tinted mineral fluid, La Roche Posay Anthelios mineral, Colorscience Face Shield, Replenix Ultimatte , and Alastin hydratint pro mineral.
                  &#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    
                    Another reason why skin sometimes looks uneven, is due to the presence of skin dullness, often due to too much dead skin cell layers that are not being shed efficiently.  The use of retinols every night, or every other night for the sensitive skin types, can help with exfoliating the dead skin cell layers over time. Think of this category of products as chemical exfoliants. The main concern with this category of products is that it can leave the skin dry and irritated if the formulation is not tolerable. It is also not suitable for those who are pregnant or nursing .
                  &#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    
                    Some over the counter options include: ROC retinol correxion, the Ordinary, Renova, and SkinBetter Alpharet.
                  &#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    
                    Ascorbic acid, or Vitamin C is an antioxidant. It protects our skin from the presence of free radicals and helps build collagen and reduces pigmentation. Vitamin C formulations are often quite acidic and those with sensitive skin need to be careful. I suggest starting at a low concentration for Vitamin C like a 10% formulation and slowly increase as tolerated gradually. There are formulations that are as high as 30% , but one must anticipate that irritation is more likely at these higher concentration.  Vitamin C when combined with other antioxidants can create even better results than Vitamin C by itself.
                  &#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    
                    Several combination vitamin C brands include: SkinBetter Alto and CE Ferulic.
                  &#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    
                    Over the counter vitamin C preparations exist that are at a lesser price point: Drunk elephant C-Firma, and Murad. 
                  &#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    
                    Arbutin is an organic skin lightening agent found in the leaves of pear trees and certain plants such as wheat and bearberry, and in lower quantities in cranberry and blueberry leaves. Arbutin is also an antioxidant in the skin. It is less potent than hydroquinone (a prescription skin lightening agent with some side effect potential), but has a better safety profile than hydroquinone.  Over the counter, it is found in the Ordinary skin line for a very affordable price.
                  &#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    
                    Azelaic acid is a naturally occurring molecule derived from rye, wheat, and barley. It is a molecule capable of multiple effects on the skin, from reducing inflammation in the skin, acting as an antioxidant, helping fight comedones and thickened skin, and reducing pigmentation. Although easily available as a somewhat pricey prescription, at a much lesser price point and reduced concentration it can be found over the counter in the Ordinary line.

  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    
                    Some did not make the list
                  &#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    
                    There are other molecules that we can discuss in future posts that can help the skin’s complexion as well. But if I were stuck on a remote island and only had room for 1 to 5 items, they would be found in the above categories ( Number one would be sunscreen of course!). If there is room for other options, I would consider adding products that contain niacinamide, exfoliating agents such as weekly peels with glycolic, salicylic, or lactic acids, hydroquinone, cysteamine,  and oral supplements such as oral niacinamide and  polypodium extract.

&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;

Thus if there is discordance between what Google says you have and what your doctor thinks you should be treated for, there is a valid reason why.

As physicians and healthcare providers we took an oath to “do no harm” and will not always agree with what is put forth on the internet when it is not backed by evidence-based medicine. The term evidence-based medicine means that clinicians use science, engineering, biostatistics and epidemiology, such as meta-analysis, decision analysis, risk-benefit analysis, and randomized controlled trials to create scientific evidence. The evidence is then translated into practice by selecting treatment options for specific cases based on the best research, patient preferences and individual patient characteristics ( Source: hopkinsmedicine.org) .

                    Rosacea is a chronic skin condition where facial blood vessels open and close irregularly and can cause persistent, frequent, and oftentimes severe facial redness. Depending on the type of rosacea, sometimes they may experience facial spider veins, acne cysts, styes, and thickened facial skin.
                  &#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    
                    Yes, there are mites on our skin that can give rise to flare ups of Rosacea. These mites like to live on the nose and cheeks where rosacea tends to flare.  Studies have found that when these mites overgrow on the skin, rosacea symptoms can be found. It is thought that the presence of too many of these mites can mount an immune response that results in typical red rosacea bumps. Many rosacea patients have large numbers of this mite on the skin, in their oil glands. However, some people who do not have rosacea can harbor these mites as well.
                  &#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    
                    Yes, niacinamide containing products are suitable for rosacea prone skin. Niacinamide is a VitaminB3, and can be useful in treating acne and rosacea. It increases the barrier function, which allows better tolerance in the skin against irritant reactions.  It also has anti-inflammatory effects, and is very suitable for sensitive, rosacea, or redness prone skin. It has also been shown to have an effect to reduce the risk for skin cancer formation when taken at sufficiently high oral dosages.
                  &#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    
                    Many things have been found to trigger worsening of rosacea, or rosacea flares. But scientists are still trying to figure out what exactly causes rosacea. In rosacea studies, the following has been found:

&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;

                    This past week the CDC finally made a recommendation that the use of CLOTH face coverings can help slow the spread of COVID-19 . When people exercise social distancing measures , but are still exposed to a certain degree to the presence of others ( such as at the grocery store, pharmacies, ) it would be wise to wear a mask to prevent the unwanted transmission of large droplets that may carry the coronavirus.

        Why did wearing a mask for the general public become a NEW recommendation ?

                    We have lots of evidence now pointing to the transmission of this virus when patients are not symptomatic, and shedding the virus happens about 48 hours before the onset of symptoms. Thus people who act and feel healthy, may not knowingly be spreading the virus via coughs, sneezes, or exhalation. That is why wearing a mask in everyone may be a helpful idea.

        What materials do I use for a homemade cloth mask?

      &lt;/b&gt;&#xD;
    &lt;/em&gt;&#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    
                    Studies comparing materials for home made masks were investigated in a paper by researchers at Cambridge University. Their study showed that for homemade masks, a two-layered cotton material or double layer pillowcase material are more superior in balancing breathability and effectiveness to capture virus particles when compared to other materials such as dish towel, vacuum cleaner bag, silk, scarf, or linen.
                  &#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;
  &lt;p&gt;&#xD;
    
                    Lastly, although the cloth mask recommendation may be a very good idea for the general public, the evidence does not seem very promising for healthcare workers to wear cloth masks at work. Several studies have pointed out the inadequate protection from cloth masks for healthcare workers. In the only prospective randomized controlled study of cloth mask vs. surgical mask vs. control group by MacIntyre et al. in 2015, the data shows a higher infection rate in the cloth mask arm over control and surgical mask group. Thus it appears that for the healthcare worker, a homemade cloth mask should only be considered as a last resort, as it would only be better than no protection at all.
